This page holds Graduate School Forms. Please visit the respective office or department website for forms relating to their area, such as the Registrar's Office Forms Page (for add/drop forms, etc.) or the Financial Aid Office Forms Page.

The publications and forms on this page are the property of the University of New Hampshire Graduate School. Forms should be downloaded, printed off, and all required information, documents and signatures supplied before submitting to the Graduate School. If you have any questions on what form to use or what is required please contact us. Note: Most of the forms are interactive PDF forms. Please fill these forms out in your browser window, then print them off before submitting them. We do not currently allow you to email the forms to us due to regulation requirements for having valid signatures. However you can also save a copy of the form to your computer and fill it out at your leisure before printing it.

Important Note: All International Applicants who wish to apply need to first submit a Preapplication Form. Those forms can be completed online here (preferred) or downloaded from this page and mailed to our office. Visa and other forms can be found at the Office of International Students and Scholars website.

	Application for Admission to Graduate Study	The main Grad School application. Used for most degrees except the MBA in Business, Accounting, and Economics. Do NOT use this form if you have already filed an online application.
	Application for Whittemore School of Business	Used for all Business, Accounting, and Economics degrees. Do NOT use this form if you have already filed an online application.
	Application for PBACC Certificate Programs (As a Primary Degree)	Used to apply for a PBACC Degree (Post Baccalaureate Certificate) in one of the UNH Certificate programs as your PRIMARY degree.
	Application for PBACC Certificate Programs (As a Secondary Degree)	Used to apply for a PBACC Degree (Post Baccalaureate Certificate) in one of the UNH Certificate programs as a secondary degree. Only use this form if you are already currently enrolled in a graduate degree program.
	Application for College Teaching	Used to apply to the program in College Teaching.
	International Applicant Financial Declaration (Fall 2008-2009 Form)	Required form to be submitted with an Full International Application.
	International Preapplication Form	For International Applicants who wish to apply to the Graduate School. This form *must* be submitted before the full application. This form is best submitted online.
	Letter of Recommendation Form	Form used in the Main Grad School Application for submitting letters of recommendation. (Envelopes not included, use regular ones instead) Print for use with online application.
	Letter of Recommendation Form - Whittemore School of Business	Form used by the Whittemore School of Business Application for submitting letters of recommendation. Print for use with WSBE online application.
	Statement of Residency	Form used by applicants to apply for In-State residency. Print for use with online application.

	Graduate School Catalog	The current Graduate School catalog.
	Change of Degree Form	Form for currently enrolled students wishing to change their degree program. (For changes within the same department only. If you want to change your degree to another department you have to submit a new application. Complete instructions are on the backside of the change of degree form)
	English Dept. Teaching Assistantship Form	For applicants to programs in the department of English who are applying for a Teaching Assistantship. This is a link to their Aid Page.
	English Dept. Financial Aid Form	For applicants to programs in the department of English applying for financial aid. This is a link to their Aid Page.
NA	Intent to Graduate Form	You can file your intent to graduate online. Please use the link provided for instructions on how to file your intent.
	Leave of Absence Request	Form to request a leave of absence from the Graduate School.
	Master Committee Form	Form for nominating Master's Supervisory Committee.
	Petition for Exception to Academic Policy	General petition to request exception to some form of academic rule or policy (such as fee waivers, time limit extension, special grade or course considerations, etc.).
	Petition for Reinstatement	Form to apply for reinstatement to the Graduate School after having degree status discontinued.
	PhD Candidacy Form	Form used to request advancement to candidacy.
	PhD Committee Form	Form for nominating PhD Supervisory Committee.
	Readmission Form	Form for readmission to the Graduate School.
	Special Credit Form (UNH Credits)	Form to request transfer of credits for UNH course work taken prior to admission to degree program.
	Thesis & Dissertation Manual	Manual of structure, policy and guidelines for completing a Thesis or Dissertation.
	Transfer of Credit Form	Form to request a transfer of credits for non-UNH coursework. Submit form with official transcript showing courses taken.
