This page holds Graduate School Forms. The publications and forms on this page are the property of the University of New Hampshire Graduate School. Forms should be downloaded, printed off, and all required information, documents and signatures supplied before submitting to the Graduate School. If you have any questions on what form to use or what is required please contact us.
Adobe Reader
Most of our forms are in Adobe PDF format. You can visit the Adobe website and download the reader for free. Most of these forms work best if you are on a fairly current version of Adobe Reader. Most form errors are related to people using very old versions of the software. It is recommonded that you upgrade to version 8.X or later.
Forms and Publications: General Information
Most of the forms are interactive PDF forms. Please fill these forms out in your browser window, then print them off before submitting them. A few forms may be links to a webpage where you can fill out and submit your information online.
- Change of Degree Form and Instructions
- Form for currently enrolled students wishing to change their degree program. (For changes within the same department only.) If you want to change your degree to another department you have to submit a new application. Complete instructions are included on the Change of Degree form.
- Doctoral Candidacy Form
- Form used to request advancement to candidacy.
- Doctoral Committee Form
- Form for nominating PhD a Supervisory Committee.
- Dissertation Survey of Earned Doctorates
- PDF version of the Survey of Earned Doctorates. All PhD students submitting a dissertation for binding must complete and submit this form with their dissertation copies for binding.
- Leave of Absence Request
- Used to request a leave of abscense from UNH.
- Masters Committee Form
- Form for nominating Master's Supervisory Committee.
- Non-Degree Credit Transfer Request (UNH Credits)
- Form to request transfer of credits for UNH course work taken prior to admission to degree program.
- Petition for Exception to Academic Policy
- General petition to request exception to some form of academic rule or policy (such as fee waivers, time limit extension, special grade or course considerations, etc.)
- Petition for Reinstatement
- Form for reinstatement to the Graduate School if your degree status was discontinued in the same semester for which you are applying for reinstatement.
- Thesis
and Dissertation Manual
- Manual of structure, policy and guidelines for completing a Thesis or Dissertation. Theses and Dissertations that do not follow this format will not be accepted.
- Thesis
and Dissertation Microfilm Publishing Agreement
- This is the link to the publishing agreement that all students submitting a thesis or dissertation for binding are required to submit. When prompted, enter 'dissertations' as the user name and 'publish' as the password.

- Transfer of Credit from Non-UNH
Institution Form
- Form to request a transfer of credits for non-UNH coursework. Submit form with official transcript showing courses taken.
- Withdrawal from Graduate School
- If you wish to withdraw please contact Dovev Levine (dovev.levine@unh.edu) at the Graduate School directly. The effective date of withdrawal begins the day our office recieves your email request for the form, unless you request an earlier effective date on the form and provide reasons for the earlier date.
- Application to Graduate School
- We have moved to an online only application process. If you are unable to use the online form you can use this PDF version of the application.
- Application for Whittemore School of Business
- For all Business, Accounting, and Economics degrees. We have moved to an online only application process. If you are unable to use the online form you can use this PDF version of the application.
- Application for Post-Baccalaureate Certificate (PBACC)
as a Primary Degree
- Used to apply for a PBACC Degree (Post Baccalaureate Certificate) in one of the UNH Certificate programs as your PRIMARY degree.
- Application for Post-Baccalaureate
Certificate (PBACC) as a Secondary
Degree
- Used to apply for a PBACC Degree (Post Baccalaureate Certificate) in one of the UNH Certificate programs as a secondary degree. Only use this form if you are already currently enrolled in a graduate degree program.
- Application for College Teaching as a Secondary Degree or Cognate Minor
- Used to apply to the Masters of Science in Teaching degree in College Teaching as a secondary degree or the College Teaching Cognate Minor.
- Application for Readmission to
Graduate School
- Form for readmission to the Graduate School. Additional documents may be required. Please contact the graduate school directly for complete details.
- International Financial Declaration Form (For admitted international
applicants only)
- This form is only for admitted international students who plan on attending UNH. It is a mandatory requirement for VISA processing. Do NOT submit this form until you have been officially admitted.
- International Preapplication Form (Required prior to submitting a full
application)
- For International Applicants who wish to apply to the Graduate School. This form must be submitted before the full application and can only be submitted online using our web form. If you are unable to use the online form please email your department of interest and provide all the answers asked on the form in your email.
- Letter of Recommondation Form (General)
- Form used in the Main Grad School Application for submitting letters of recommendation. (Envelopes not included, use regular ones instead) Print for use with online application.
- Letter of Recommondation Form (Whittemore School of Business)
- Form used by the Whittemore School of Business Application for submitting letters of recommendation. Print for use with WSBE online application.
- Public Health - Essay Questions
- The Masters in Public Health application requires additional essays above and beyond what is required of all graduate school applicants.
- Residency
Form (Proof of Residency)
- Form used by applicants to apply for In-State residency. Print for use with online application.

- Dissertation Fellowship Application
- Dissertation Fellowship Applications are only accepted during the open application date range. When those dates are active the instructions page will be updated to the current academic semester/year and any changes needed to the forms and process will also be updated.
- English Department Financial Aid Application
- For applicants to programs in the department of English applying for financial aid. This is a link to their graduate financial aid form.

- For applicants to programs in the department of English who are applying for a Teaching Assistantship. This is a link to their graduate financial aid form.
- Faculty Fellowships (Summer)
- These awards are intended primarily for non-tenured, tenure-track faculty members for whom summer support can significantly contribute to professional growth and achievement. Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
- Faculty Mentor Award
- This award is designed to honor a faculty member whose commitment to excellence in graduate student training has contributed significantly to graduate students’ professional development. Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
- Graduate Assistant Handbook

- Graduate Research/ Scholarship/Creativity Award
- This award is designed to recognize graduate student research/scholarship and other creative works that represents the best overall contribution to the discipline. Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
- Part Time Scholarship Application
- Form to apply for a Part Time Scholarship through the Graduate School. Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
- Summer Teaching Assistant Fellowship
Application
- Forms to apply for a Summer Fellowship for Teaching Assistants (Includes cover letter, deadlines and recommendation form). Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
- Teaching Assistant Award
- Graduate Teaching Assistants contribute significantly to the teaching mission of the University. These awards are designed to recognize graduate students who have demonstrated excellence in teaching. Forms only accepted during application period. - Catalog, Graduate School
- As of September 2009 the Graduate School catalog is only available online. We do have our old 2007-2009 catalog available in PDF format, for review purposes only as much of the information has changed.

Remember ...
- To follow ALL DIRECTIONS exactly when using our forms.
- Many forms let you fill in data before printing - just be sure to print the form before you close it as the data you enter will not be saved in it.
- A few forms will let you enter data and then email that data to us as an email attachment.
- Some forms are actually links to online interactive forms you can submit from a website.
