This page holds Graduate School Forms. The publications and forms on this page are the property of the University of New Hampshire Graduate School.  Forms should be downloaded, printed off, and all required information, documents and signatures supplied before submitting to the Graduate School. If you have any questions on what form to use or what is required please contact us.

 

Adobe Reader

Most of our forms are in Adobe PDF format.  You can visit the Adobe website and download the reader for free.  Most of these forms work best if you are on a fairly current version of Adobe Reader.  Most form errors are related to people using very old versions of the software.  It is recommonded that you upgrade to version 8.X or later.

 

 

Forms and Publications: General Information

Most of the forms are interactive PDF forms. Please fill these forms out in your browser window, then print them off before submitting them. A few forms may be links to a webpage where you can fill out and submit your information online.

 

  • Academic Forms (Committees, Petitions, Change of Degree, Graduation and Intents, etc.)
  • Change of Degree Form and Instructions
    • Form for currently enrolled students wishing to change their degree program. (For changes within the same department only.) If you want to change your degree to another department you have to submit a new application. Complete instructions are included on the Change of Degree form.
  • Doctoral Candidacy Form
    • Form used to request advancement to candidacy.
  • Doctoral Dissertation Committee Form
    • The Doctoral Dissertation Committee should be nominated after the doctoral student reaches candidacy.
  • Doctoral Guidance Committee Form
    • The guidance committee should be nominated by the program faculty as soon as possible after a student has begun study for the doctoral degree.
  • Dissertation Survey of Earned Doctorates
    • PDF version of the Survey of Earned Doctorates. All PhD students submitting a dissertation for binding must complete and submit this form with their dissertation copies for binding.
  • Leave of Absence Request
    • Used to request a leave of absence from UNH.
  • Masters Committee Form
    • Form for nominating Master's Supervisory Committee.
  • Petition for Exception to Academic Policy
    • General petition to request exception to some form of academic rule or policy (such as fee waivers, time limit extension, special grade or course considerations, etc.)
  • Petition for Reinstatement
    • Form for reinstatement to the Graduate School if your degree status was discontinued in the same semester for which you are applying for reinstatement.
  • Thesis and Dissertation Manual
    • Manual of structure, policy and guidelines for completing a Thesis or Dissertation. Theses and Dissertations that do not follow this format will not be accepted.
  • Thesis and Dissertation Microfilm Publishing Agreement
    • This is a PDF of the UMI publishing agreement.  All students completing a thesis or dissertation, with the exception of MFA students, must submit this form to the Graduate School at the time their final copies are brought in to be bound.

  • Thesis and Dissertation Submission Checklist
    • Checklist for use when submitting final copies of thesis/dissertations to the Graduate School for binding.
  • Transfer of Credit from Non-UNH Institution Form
    • Form to request a transfer of credits for non-UNH coursework. Submit form with official transcript showing courses taken.
  • Transfer of Non-Degree Credit from UNH Form
    • Form to request transfer of graduate level non-degree credits for UNH course work, taken prior to admission, to a graduate level degree program.
  • Withdrawal from Graduate School
    • If you wish to withdraw please contact Dovev Levine (dovev.levine@unh.edu) at the Graduate School directly. The effective date of withdrawal begins the day our office recieves your email request for the form, unless you request an earlier effective date on the form and provide reasons for the earlier date.
  • UNH Law Graduate Exchange Approval Form
    • UNH students must complete a short application form for UNH School of Law before they may register. Likewise, UNH School of Law students must complete a short application form for UNH before they register. UNH students will register both in a placeholder course at UNH and in the actual course at UNH School of Law. UNH School of Law students will register for the actual course at UNH. Normally, no more than one course (3 credits at UNH School of Law and 3 or 4 credits at UNH) can be taken by any one student in any one term.

  • Admissions (Applications, Readmission, Residency, Letters of Reference, etc.)

 

  • Graduate Aid (Aid Request, Scholarship/Fellowship Applications, Awards, etc.)
  • Dissertation Year Fellowship (DYF) Application (closed)
    • Dissertation fellowships for a maximum tenure of one academic year are available on a competitive basis to doctoral students who have been advanced to candidacy. These awards include a stipend and a waiver of the doctoral research and mandatory fees for the period of the award. Forms only accepted during application period.   • English Department Financial Aid Information and Applications
    • This link is for anyone applying to a program in the department of English and are requesting financial aid from the department.
  • Faculty Fellowships Summer (closed)
    • These awards are intended primarily for non-tenured, tenure-track faculty members for whom summer support can significantly contribute to professional growth and achievement. Forms only accepted during application period.
  • Faculty Mentor Award
    • This award is designed to honor a faculty member whose commitment to excellence in graduate student training has contributed significantly to graduate students’ professional development. Forms only accepted during application period.
  • Graduate Assistant Handbook
    • Policy and procedure manual for graduate assistants
  • Graduate Research/ Scholarship/Creativity Award
    • This award is designed to recognize graduate student research/scholarship and other creative works that represents the best overall contribution to the discipline. Forms only accepted during application period. When application dates open the form will be updated to the current academic semester/year.
  • Part-time Tuition Scholarship Applications (closed)
    • Form to apply for a Part Time Scholarship through the Graduate School. This scholarship provides a partial tuition waiver for students. Forms only accepted during application period.
  • Summer Teaching Assistant Fellowship (STAF) Applications (closed)
    • STAF fellowships are intended to support research or study during the summer for individuals who have held a TA position during the same academic year, and have performed exceptionally well as a TA and as a student. Forms only accepted during application period. See the Summer Teaching Assistant Fellowship Instructions page for how to apply.
  • Teaching Assistant Award
    • Graduate Teaching Assistants contribute significantly to the teaching mission of the University. These awards are designed to recognize graduate students who have demonstrated excellence in teaching. Forms only accepted during application period.
  • Travel Grant Request Form
    • Travel grants are available to assist graduate students presenting papers and poster sessions at professional meetings and/or conferences. Travel grants will not exceed $200 per student per conference or $400 per student per year. Students receiving travel grants will be expected to participate in the Research Day graduate student appreciation event held each Spring, for the academic year that they receive the travel grant.

  • Catalog, Graduate School
    • The Graduate School catalog is only available online.  We do provide archives of the catalog, and course descriptions, on our graduate school archives page.
  • Demographics
    • Report of general demogrpahics, based on fall semesters, for UNH Graduate Students.  Updated each fall and provided in Adobe PDF format.
  • Dissertation Titles
    • PDF report of dissertation titles for our graduate students.  Updated each fall.
  • Thesis Titles
    • PDF report of thesis titles for our graduate students. Updated each fall.

  • To follow ALL DIRECTIONS exactly when using our forms.
  • Many forms let you fill in data before printing - just be sure to print the form before you close it as the data you enter will not be saved in it.
  • A few forms will let you enter data and then email that data to us as an email attachment.
  • Some forms are actually links to online interactive forms you can submit from a website.
